No, I didn't miss the "prior to 1970", but I was at a loss as to the identity of the person. Any of your last five clues would have led me to the correct answer.

4. He worked with two of his brothers, Jerry and Moses, but there is only one brief example of him working Jerry.

That is an EXCELLENT guess Dave, but incorrect. This person died of a heart attack, prior to 1970, when they were only 60, unlike Jack who passed away just last year at 90. Thanks for showing interest. Here are some more hints besides the one I just let out of the bag.
1. His real name was Samuel, but he went by a stage name.
2. He was from a family of entertainers.
3. Happened to die while doing something he feared.
I know these aren't the best hints, but in this day and age when it is so easy to look things up on the internet I don't want to be too specific.
And since Dave is the only person to show any interest, so far, I'll give the answer if he asks for it or I'll post it in a few days if he doesn't.
