Nip/Tuck - who was the Carver?

Hi,
the episode ehere the Carver was revealed was shown here in Sweden a couple of weeks ago. Unfortunately I was out of the country at that time, and it seems as if no one I know watches the show, so I wonder who the Carver was.

And one more thing, if he (my guess is a she!) was not in the first two seasons, please also explain WHO he is, since I haven't really been following the series lately.
